Disarm Glitch, permanent loss

So a character I have is a mix of rogue/SD/fighter.

I had a standard, low level bronze rapier, offhand short sword while adventuring in the Mound (yuan-ti). I got disarmed by the NPCs and my mainhand disappeared. Not just from my hand, but also inventory. It didn't fall on the ground, or onto any body. The hotbar I had my weapons setup on cleared. It's poofed.

I'm glad it's just a bronze weapon but... this could lead to other weapon losses if it persists.
I had more than enough inventory space, so a full inventory wouldn't be the reason for this weapon up and poofing either.
