Warlocks can still cast eldritch blast in chicken form (may be intentional given warlocks can blast in other forms).

Resting removes the curse (though noting the spell being cast even on yourself puts you in combat).

I cannot reproduce this. Please clarify when and where this is occurring, what steps are taken, the subject this is occurring on, etc.

Would need to test a bit in game, but in this specific circumstance - I had self-cast polymorph (cow), and had Remove Curse cast on me (from a scroll) a couple times. It didn't remove the shape- though I'm wondering if I had to find it on the radial menu somewhere akin to shapeshift and cancel it after the fact? Apologies for the lack of clarification there.
