To apply to the DM team to make a type of character that would require such an application, what template would i use, and to whom should i send the application?

In my experience, applications are sent as forum PMs to 'Active DMs'. As for formatting, just do your best to touch on the points needed for the application in a format you feel fits.

Pretty much.
As a rough guidline I'd go:
Forum PM go Active DMs
*name of character/or log in and the app you wan't (noble, good monster, ect)
Then in the body of text:
*Ideally the name of the character - if you've not decided that then your log in
*What you are applying for
*A little backstory (Note this doesn't have to be a 390 page novel. Just a paragraph or two is fine.
*A bit about the characters personality/drives/methods/stuff like that - who they are.
*Consider how your app relates to the character, tie that in with the above.
For Specific Awards/PRCs consider
*With Good monster races, consider how you're going to roleplay the dictomony of being a good monster in an evil setting (the underdark) and what you might do if you're 'found out' as someone with morality. What your plans are for such an eventuality
*With a NPC Noble House app - consider how your character fits in (or doesn't!) to the noble house in question. Make sure to touch on that.
*Pirate UD race - consider how they chose a life of surface piracy, over the underdark.
I think that covers most of the normal awards? Someone correct me if I'm wrong.
Oh.
Bonus Content: - If you're apping for An assassin? You'll also want to add a Sample Assassination - writing up an example of an assassination job your pc might do.

In addition to what GrumpyCat noted above, for "organization" class applications (Harper, Zhent, assassin) keep in mind two questions:
Why would the organization be willing to recruit this character?
Why would this character be willing to be recruited by the organization?
