I've only recently remembered this feat exists. It's an extremely uncommon choice because poison and disease are little more than a minor inconvenience for characters eligible to select this feat, and here is why:
  • 21 to 22 CON is the maximum most builds aim for.
  • 25 CON already equals a huge boost to Fortitude saving throws - in the majority of cases, poisons and diseases would therefore only apply when rolling a 1.
  • Great Fortitude, the prerequisite feat, is absolutely superfluous for such builds
My suggestion would thus be to change the requirements to something more reasonable, like 18 CON and no feat. Furthermore, characters with 21+ hard CON could be granted Perfect Health for free.

Additionally, to make it an epic feat, characters with Perfect Health could gain a minor amount of regeneration (+2, perhaps) and lose a random negative status effect with every server tick. This seems like a decent quality of life upgrade and would better represent their epic resilience without having a huge impact on balance.
