I've only recently remembered this feat exists. It's an extremely uncommon choice because poison and disease are little more than a minor inconvenience for characters eligible to select this feat, and here is why:
Additionally, to make it an epic feat, characters with Perfect Health could gain a minor amount of regeneration (+2, perhaps) and lose a random negative status effect with every server tick. This seems like a decent quality of life upgrade and would better represent their epic resilience without having a huge impact on balance.
- 21 to 22 CON is the maximum most builds aim for.
- 25 CON already equals a huge boost to Fortitude saving throws - in the majority of cases, poisons and diseases would therefore only apply when rolling a 1.
- Great Fortitude, the prerequisite feat, is absolutely superfluous for such builds
