Item Generation Bug?

A video game instructed me to post this:
Found in Baator, at the Lethe. On an undead (can't recall what their name is, but there's only one sort there anyways).

I have similar item. Found in RDI, within the lower Dragon lair chest.

Found a belt with the same description at the end of the Maur dungeon.
If I recall it was in the second chest from the left if you're facing the default direction after exiting the cave (facing towards the big empty inaccessible area).
