The derro in the Derro Tunnels are currently equipped with weapons that are +1 or greater making Ghostly Visage pointless to use. However, Ghostly Visage is a good measure of counterplay against the prevalent stun traps in the tunnels placed near mob spawns. Also, the Derro Berserkers hit relatively hard for the intended level.
It would be nice to see Ghostly Visage usable in the cave so you don't dread a possible one-round-kill at an early level.
