Portal lenses.
The game keeps randomly eating portal lenses and not even attempting to transfer me across servers. It doesn't even send the "attempting" message. 3500gp is already too much. Having that good poof into oblivion in an unpredictable manner is no fun.

I believe if you immediately log out of your current server and onto the server you should be on without doing anything in-between first it should transfer you to the correct portal. Sometimes it doesn't work, but worth a shot. Not saying that's a fix, but a workaround.
