I've noticed that certain wild mage surges, like combat ready and spell resistence don't always give you the mechanical changes. I'm not sure why this is, but the spell triggers, the animation shows with the text, but then nothing happens.
I've seen this so far happen with Ghost Form, Spell Immunity & Combat Ready when it's being fated. I'm not sure if there is a hidden cool-down, or something?

This is probably because of some NWN spells don't trigger correctly and gives an unknown target or similar. One of the culprit spells is Mage Armor, it will almost never work with a surge. There are other spells too, I forget which.
