Loremaster Scroll Conservation Bug

When I use a scroll and trigger conservation, I sometimes don't get the scroll back. I've checked my scroll case too, but they don't show up there either. It won't say "Acquired Item: <Scroll>" either in those cases.

Description

Despite triggering the scroll save, sometimes a new scroll is not returned to the character.

Steps to reproduce
----------------------------
1. Acquire scrolls of mage armor
2. Cast scrolls until Loremaster scroll conservation triggers

Expected result
-----------------------
I lose a mage armor scroll, but get a new one.

Actual result
-------------------
I simply lose the scroll.

For the animal scrolls, I appear to get back scrolls with a different ID. I'm not sure why. Like if I use an "Endurance" scroll, I will get back "Bear's Endurance" instead and they do not stack.

I assume you have tested this on PGCC, yes?

No, it's on live with the PC I'm playing. Mage armor doesn't return, and most of the animal spells give a different scroll. I haven't tested with barkskin or other standard buffs.
