Steps to reproduce the Bug: Random, but I'll keep documenting it. Seems to happen both with lenses bought from a pc shop and lenses bought from a NPC shop, without following any pattern.
Expected Result: Teleporting to destination.
Actual Result: The lense is consumed, but nothing happens.

)Additional Information/Notes: Probably not connected to specific areas or vendors, but happened:
With lenses bought from "Chen", goblin npc in the hub, in two separate instances. Affected areas:
- Andunor, the Hub
- Andunor, Deep Gate
- Andunor, West Wheel
With lenses bought from pc shops: Ishta's Offerings, Boreal Keep. Cannot remember in which area the lenses bugged in.
Logging out and logging directly into the target server (ex, lens bugs while trying to head to "Sibayad Outskirts" from Andunor -> Log out -> Log into Distant Shores) seems to actually teleport you to your destination. Moving to a different area (Slum Caverns) also solved the problem the first time over.
