Bug with Haunted Falls writ in the underdark

Part two of the writ requires you to enter the lower cavern, but the door puzzle to enter the lower cavern is broken, some of the levers to unlock the door to proceed are just not there. So the puzzle is currently impossible to solve which makes the writ uncompletable.

The puzzle does work as intended. This has been successfully done by myself not too long ago.
You may just be doing it wrong.

how long is not too long ago, because if you tried to do it about 2 hours ago its not possible to complete, a mentor also came in and said that some of the levers to solve the puzzle were missing

I've been travelling in this dungeon quite often by myself, and the last times I noticed that in the first half of the third corridor (the four-leafed clover), most times the main lever buzzes (damaging you) without making the other levers appear.
The times I've been in the dungeon I was alone (that I am aware of), and until level 17 I could complete without issues.
Since level 18 I can't trigger the puzzle anymore, although I don't think it's a level issue.
If there is a secret condition that must be satisfied or a tile that must be stepped on, it is not really clear, I am sorry.
