I found a transition point that when I tried to use it, returned an error that says " Error: target server does not exist ".

It is in Brogendenstein: (Beneath the Mountain) Ice Caves. It is the rope going upwards that is near the center of the map.

No longer receiving errors for this one.

Should be good going forward.
