Lost/disappearing Enchanted Greensteel Scimitar with Perm Essence

Howdy,
Description
What: Really quite unsure what happened here. Popped briefly away from my computer to make a coffee, I returned less than five minutes later and quickly realised my primary weapon was missing from my inventory.
A Greensteel Scimitar, with +1d4 Divine Enchantment and +4 Positive Permanent Essence.
In a mad panic I ran around the nearby merchants thinking I must have sold it by accident, having just returned from a writ and unloaded my loot. I quickly realised that wasn't the case and checked through my combat log to try and pinpoint when I lost the item. Finding the following:
Not sold. No interaction, just *poof*
Who Ravan'Vel
Where Bendir Tavern, upstairs near the merchants & Bank
When Approximately 20:30 GMT on 08 February
Why? I have no idea.
Steps to Reproduce
No idea.
The only vague theory I can even think of is that while leaving the dungeon (Malarites) Ravan's inventory was full. He unequipped his scimitars and it prompted the "Your inventory is full" message, but I caught this at the time and made sure I had not lost either one of his scimitars. Re-organised a couple items in his inventory and made sure it wouldn't happen again, then continued on.
Is it possible something about this script mis-fired at the next hourly/rp tick?
Per the screenshot above, he was already back in town - had sold everything and even deposited the money back into his bank account when the scimitar disappeared. This happened long after his inventory was no longer full.
