Whether or not you are disguised persists between servers.
However, if you use multiple disguises, the name of the last disguise used on that server is what is used.
Example:
In Shadovar Trade Post (surface server), I am disguised as Ashley. I transition to Shadow Wharftown (city and planes server).
In Shadow Wharftown, I turn off disguise and then disguise again as "Heather."
Without dropping disguise I transition back to Shadovar Trade Post. I am now "Ashley" even though I should be "Heather".
