Loot Item bug

Description:
Killed the Greater WW in the Forest of Despair and looted the body and found a pair of Boots labled: Tier2m generated loot item.


Steps to reproduce:
Tried killing the Greater WW again, but no magic item second time.

Expected result :
Get a normal magic item

Actual result
Got a pair of Boots labelled: Tier2m generated loot item on the first attempt

I got a bugged item as well on the surface server, in the kobold mines near Cordor. Im not shure what im doing wrong but i cant seem to upload the image so i will just write down the text in the item:

On the (Underdark Middle) Dolgom of the deep giants map, and from the Armor Locker a Belt with the title tier2m generated loot item was found. The description says to report it here if you see this name.

Found this item in the Haunted Cave outside Saltspar (the unsolvable one) in the chest on your right as you enter the first room with chests.

Steps to reproduce
----------------------------
The first time I thought maybe it happened because there was already a cloak in there when tier2m spawned, but I found another one in the same chest when there wasn't already a cloak in it.

I can't remember the precise name of the area, but it happend on the second level of the... memory thief place in Shadow Plane, from the boss room chest.
