Leader heads not spawning on bosses

So lately in the past week or so I've been through dozens of dungeons and about 95% of the time there are no heads on all the bosses that I kill. I've asked around and it seems that there arn't other's that are experiencing this. Perhaps it's just me. :|.

Do you have head bag? I often find I forget I had head bag and then realize it's full of heads.

Yes I do. But it's not going there. Unless I have some invisible headbag somewhere.
