The size bonus is invisible, but of course it is invisible in the first strike as well, meaning that 19 is with size all right.
Int 18 means mod 4, and since it increases by that amount, 19 + 4 = 23.
It might not have anything to do with size, but nevertheless, it's wrong.
The regular attack with 19AB includes the size bonus in the attack roll rather than lowering the target's AC by 1. It's basically 18+1. Meanwhile, Elemental Strike with 22AB doesn't
show this +1AB, but rather reduces the target's AC accordingly. It is therefore exactly 4 higher than the regular attacks would be if the size bonus was invisible for them (22 = 18 + 4).
In case anyone's curious why I didn't simply increase the AB by one: It's because, according to my testing, the attack bonus of regular attacks displayed in the combat log doesn't increase when attacking a dragon as a medium-sized char. If Elemental Strike increased AB based on size rather than (effectively) lowering the opponent's AC, you'd suddenly see Elemental Strikes at 6 higher AB than your usual attacks (rather than 4), and we'd possibly get a similar bug report titled "Spellsword ele strike AB too high".
