Number of undead is based off spell cast. Check out undead summoning part of the wiki, it explains it clearly.

Cast mummy dust as a necromancer specialist using the skeleton stream.
Expected result
-----------------------
2x Skeletal Lord 2x Skeletal Champion/Archer Champion
Actual result
-------------------
1x Skeletal Lord 2x Skeletal Champion/Archer Champion

lvl 30 wizard, necromancer specialist, shadow mage path.
The bug seems to appear when the specialist is about to get 2x tier 7 undead.
It does not happen every time, but I have been able to reproduce it.
