There exists level drain effects in PvE content (that are also available in PvP if you know how to access them) that cannot be removed by any means whatsoever.
Steps to reproduce
----------------------------
1. Be level drained. In this example, we'll say you were leveldrained by a Spectral Krenshar summoned by a Malarite Priest.
2. Attempt to remove it with Restoration.
3. Attempt to remove it with Greater Restoration.
4. Attempt to remove it with -pray.
Expected result
-----------------------
At least one of the above steps SHOULD remove the effect, especially if done multiple times.
Actual result
-------------------
2 -> No removal.
3 -> No removal.
4 -> No removal.
