In the Bramble Woods, there is a location where one can get stuck and not be able to escape without the use of a portal lens or similar teleportation item/spell.
Starting from the Cordor Farmlands (at the exit with the mine entrance), head into the Bramble Woods and immediately go left/west. There's a raised area with a gem deposit and a climb spot on the lower side... but no climb spot on top, meaning you're effectively stuck up there unless you can portal out or starve to death. Using -makesafe has no effect, and ropes/lassos have nothing to attach to.
If there is a way to get down, I certainly didn't see it (and I'm not using any hakpacks or overrides to furniture/world objects/etc. that would hide it either).
