Disappearing Shield

It's the second time I get the message "Lost item: Tower Shield (Steel)" while I exit the crafting menu at a forge.

First time happened yesterday at the anvil in Dunmarle Castle, finished creating a helmet, the shield was equipped and after closing the crafting menu I got the message and the shield disappeared.

Second time happened not even 30 minutes ago, the forge at the rangers camp in Skal, finished creating an armor, shield still equipped (which I now know is a mistake I won't make again xD), close the menu, got the message "Lost Item".
It was equipped so now one stole it, it was fully charged (since I had to make a new one) so it did not break. No idea why I keep destroying shield, must be my char is too dumb and leaves 'em in the forge to melt XD

Anyway, I'll make another no biggies, but I wanted to report this tiny little bug.

Very late reply, I'm afraid. I'd have some suspicions if this happened when your inventory was full or nearly-full (no room to equip your shield if you were to unequip it), but otherwise this is a very strange report. Has it ever happened since you originally reported it? Do any screenshots exist?
