Every time I attempt to server transition, I have to click on the transition twice. The first click results in a portalling failure message, while the second click succeeds to move me to the other server. I also have similar issues when I try to use the greater spell focus transmutation ability to teleport to another server. Since I always fail the first attempt to move to another server, this means that when I want to change server through teleportation, I automatically trigger the 10 minute cooldown and also fail to teleport.
This happens on every server. For example, when I attempt to walk through a transition or teleport from cities and planes to surface, the exact same happens when I try to walk through a transition or teleport from surface to distant shores.
I've seen other players cast teleport and successfully change server without this issue.
Steps to reproduce:
1) Walk through a server transition.
2) Cast teleport and select a portal source located on another server.
Expected result:
1) Move to the other server.
2) Move to the selected portal source on another server.
Actual result:
1) Receive failure message and remain on the same server.
2) Receive failure message, remain on the same server and trigger teleport ability cooldown.

1) and 2) represent two different situations. 1) is related to the server transition bug while walking and 2) is related to the server transition bug while using the GSF transmutation teleport ability.
The failure message seen in the above screenshot is the same message I receive when I walk through a server transition. Somehow the second time I click on the server transition I no longer receive the failure message, and move to the other server. So I imagine that if I could cast the teleport ability again after failing the first attempt, I could actually teleport. The problem is the 10 minute cooldown, which triggers after the first attempt, making impossible any further attempts within 10 minutes.
It should also be relevant to mention that the success between the first and second attempts seems to have a short timer. If I click again on the server transition right after failing the first attempt, I can change server. But if I click on the transition a few minutes after the failed first attempt, I receive the failure message yet again.
