Description

When a player dies and someone tries to use a Resurrection Rod, scroll, raise dead or elk stone, nothing happens. All situations for it to work are present, piety, player online, same server, etc.
Steps to reproduce

Have a PC die. Present the fresh corpse. Someone tries a method to raise them.
Expected result
-----------------------
Piety is removed from caster, item is deleted, dead player is prompted, dead PC is raised.
Actual result
-------------------
Attempts with various scrolls, rods, etc., nothing happens. No prompt to the player to say Yes. Piety is removed from the caster, item used is deleted.
Misc Notes
----------------
Tried five times with various scrolls, stones, rods. Picked up, put down and shifted the body around. Then attempted a log off then back on. Attempted with both dead PC and caster jumping to a different server (attempt 6, and this worked).
Another person died and a raise dead didn't work, but then a second attempt worked with another caster.

So just to clarify for my own narrowing it down purposes:
Was the person afk during any attempt?
Did they get the conversation and click no at any stage?
Be careful with picking up the body too early as well, it force closes the conversation that person is in and thus may cancel the raise attempt.
Shrines and altars have always been weird and actually only have a chance per 6 seconds to work.
As for spells:
You used raise and resurrection as a caster?
As for items:
I'll take a look see why they may be being ignored or non functional.

Both players were present and communicating they weren't receiving the prompts.
No actual clerics were used in the making of these attempts. It was all items and one shrine.
For death #1:
A raise dead scroll was used, then a resurrection rod and another raise dead scroll, before they received a prompt and were successfully raised. (I think the rod and scroll were used simultaneously, but both items were deleted from each person's inventory).
For death #2:
The body was picked up and dropped, then two attempts to raise were made.
Nothing happened and the body was move again and another attempt was made.
Two minutes passed and still nothing so they tried logging off then on again. (Double checked my logs
)
Two more attempts were made and no prompts. Another two minutes passed...
Someone had a last resort idea and left the dungeon with the body to switch servers. They used a shrine and this worked.
The failed items used were . . .two from a resurrection rod, one raise dead scroll, one resurrection scroll, one White Elk Spirit Stone, by four different people.
I was told these issues have been going on for a while, that scrolls and rods have been failing to raise / prompt the dead person for a Yes or No.

Thank you for the added information!
I'll give it a good look with each of these scenarios, it may be that items are acting differently, whereas I had believed they were not, and for shrines it may be another mechanism entirely.
I suspect it's just crossed wires with items then and something I did to stop shrines spamming the conversation.
